A predisciplinary conference was held on (date) in my office and you presented no new information to change the proposed course of this action.  

 

Mr. Smith and I came to your office and found you at your desk with your head resting on your arms and snoring on October 22,  200X, around 4 pm.  This is the second such incident in the past 4 months. Sleeping on duty is considered unacceptable personal conduct, specifically conduct for which no reasonable person should expect to receive prior warning and the willful violation of known, written work rules.
